Conservative Prosthetic-periodontal Treatment for Molar Class Iii Furcation Involvement: Case Report
نویسندگان
چکیده
Purpose: The treatment for furcation involvement is a great challenge to the general dentists. Tunneling may be a treatment alternative for class II and III furcation involvements in mandibular molars with large angle separation and great divergence between the mesial and distal roots. This alternative is a conservative treatment that allows a great condition to oral hygiene maintenance by the patient. Thus, the aim of this case report was to describe a conservative and therapeutic treatment modality for the horizontal defect of periodontal tissues in the furcal area with buccal-lingual extension (class III furcation involvement). Case Report: A patient with class III furcation involvement in the first mandibular molar was submitted to root resection and periodontal surgery to expose the clinical crown for the full-coverage restoration with tunnel preparation. Results: The final result of the treatment with tunneled crown was favorable and predictable due to adequate hygiene condition to avoid plaque accumulation and occurrence of root caries. Clinical Significance: A multidisciplinary approach is essential to achieve a correct treatment plan including surgical-periodontal procedures integrated to the prosthetic rehabilitation. Besides, the recommendation for oral hygiene maintenance is essential for the treatment longevity with tunneled crown.

Compromised teeth often pose a significant challenge to the clinician, complicating the treatment plan and compromising long term prognosis. Upper molar teeth showing periodontal attachment loss, caries or fractures involving the furcation area are prime examples of such challenges. Treatment option for affected teeth can be roughly divided into maintenance of the tooth through root resection t...

Advanced furcation invasion has long been a treatment dilemma for the clinician. The present two case reports used a glass ionomer restorative material as a barrier in the treatment of maxillary Class III furcation defects. One year follow-up results showed a reduction in tooth mobility and probing depths and no bleeding on probing with the use of the glass ionomer. These case reports offer ano...

Purpose Furcation involvement in the molars is difficult to treat, and has been recognized as a risk factor for tooth loss. Although periodontal regenerative therapies, including guided tissue regeneration and various types of bone grafts, have been applied to furcation defects, the effects of these treatments are limited, especially in large class III furcation defects. The purpose of this pil...

Endodontic therapy has a high survival rate, but in cases of post-treatment pathosis, retreatment may be required. This case report presents a similar case of endodontic failure which resulted in a coexistent periradicular-periodontal lesion. The case was managed by nonsurgical endodontic retreatment followed by photodynamic therapy in the periodontal region. Satisfactory results were obtained ...

Pulpal and periodontal problems are responsible for more than half of the tooth mortality. There are some articles published in the literature about this issue. Many of them are quite old. There has been also lack of knowledge about the effect of endodontic treatment on the periodontal tissue healing and suitable treatment interval between endodontic and periodontal treatments. In this case rep...
